Healing Our Village of Maryland, Inc. (HOVMD) will be providing Annual Wellness Exams to facilitate the completion of a Disability Benefits Questionnaire (DBQ). The DBQ is a form that helps the Department of Veterans Affairs (VA) evaluate a veteran’s disability claim. DBQs are used to collect medical evidence that connects your current medical conditions to a possible service-related disability. DBQs can help speed the processing of compensation and pension claims.. There are over 60 DBQs including conditions like PTSD, hypertension, heart conditions, diabetes, skin disorders, etc.

HOVMD offices hours every Monday and Wednesday from 12 noon to 5 pm by appointment. Virtual appointments are available Monday- Friday by appointment only.
HOVMD Concierge Medical Services and KC Cardiology Services have combined forces to provide Wellness Visits, Medication Refills, Health Education Classes, laboratory services through our Mobile Health Equity Unit. Serving both urban and rural area of Georgia we strive to provide a state of the art, easily accessible care that will reduce emergency room visits. Our Mobile Medical Van is equipped with two exam rooms, EKG, Laboratory Services, and tele-medicine access to specialists throughout the United States.
HOVMD provides Specialized Veterans Medical Services to assist with disability claims. HOVMD has a team of licensed, experienced medical practitioners. We have primary care providers (PCP), cardiologists, nurse practitioner, clinical pharmacists, certified medical assistants, phlebotomists, psychologists, etc. Our primary care practitioners are experience in writing VA nexus letters and disability benefits questionnaires (DBQs). For the NEXUS letter we provide an in-depth review of your medical records including your medical service records so we can write a comprehensive Nexus Letter for your claim. We work with our Claims Specialists to make sure we provide the specific medical information that will boost your chances of qualifying for the highest percentage of service connected disability.
